Position Information:
Supv Case Management assists with managing the day-to-day operations of the department works with Patient Case Management leadership and the Patient Case Management Physician Advisor to carry out department goals.  Maintains clinical competency and serves as clinical resource to staff of the Patient Case Management Department and to the physician and nursing leadership of WakeMed.  Collaborates with physicians nursing members of the interdisciplinary team Patient Financial Services Patient Access and other departments to ensure that services are being provided in a timely/quality manner and act as a resource when resolving barriers.

Experience Requirements:
3 Years Clinical - Case Management Required - Or Clinical - Discharge Planning Required - And 1 Year Leadership Preferred

Education Required:
Bachelor's Degree Nursing Required - Or Master's Degree Social Work Required - And Master's Degree Nursing Preferred

Licensure/Certification Requirements:
Registered Nurse Required - Or Equivalent Education and Experience And Certified Case Manager Required - Within 24 Months of Hire Or Accredited Case Manager Required - Within 24 Months of Hire

Hours of Work:
M-F 8am-4:30pm

Weekend Requirements:
As Needed
