Position Information:
The Medical Assistant Population Health understands the needs of the practice and supports the mission and values. Medical Assistant Population Health (MAPH) works interdependently with other medical professionals to help manage the health and wellness of patients. MAPH will assist in the care coordination of patients with chronic diseases those who have been recently hospitalized or discharged from the Emergency Department and other patient populations as appropriate. Uses motivational interviewing to discuss services due with patients based on standing orders and quality measures guidelines. Follows up on previous orders and consults with primary care providers. Documents data from new patient records and specialist reports. Updates previous medical social and surgical history from consults emergency department and hospital reports. Ensures follow-up if appropriate to close care gaps. Enters data in discrete and appropriate fields. Work with vendors and external partners to download beneficiary utilization and quality data on a monthly basis. Uploads quality metrics to external partners. Must have a commitment to excellent written and oral skills strong organizational and analytical skills. Performs administrative duties (including but not limited to typing mail processing ordering supplies answering phone calls scheduling meetings). Other duties as requested.
Experience Requirements:
2 Years Clinical - Medical Assistant Preferred
Education Required:
High School Diploma or Equivalent Required - And Graduate Medical Assisting Preferred
Licensure/Certification Requirements:
Certified Medical Assistant Required - Within 24 Months of Hire Or Registered Medical Assistant Required - Within 24 Months of Hire
Hours of Work:
Monday-Friday
Weekend Requirements:
No Weekends
Call Requirements:
None
