Position Information:
The Patient Navigator Case Manage is a Registered Nurse responsible for providing program development and patient centered navigation & transition coordination services to select populations. Provides guidance to patients and families and facilitates communication coordination and collaboration with interdisciplinary team members to optimize outcomes across the continuum of care. Collaborates with interdisciplinary team to develop and implement tools resources protocols and evidenced based practice clinical pathways to identify at risk patient populations and coordinate services in accordance with regulatory guidelines. Serves as a patient/family advocate and acts as a communication link between physicians patients family health care team members and community resources to reduce redundancy and fragmentation of care. Anticipates and monitors potential delays in the health care continuum and acts proactively to support timely and smooth progression of the patient through the health care system and connect to resources in the community. Reviews monitors and reports relevant quality performance metrics / patient outcomes and facilitates team communication rounds and reports among providers. Initiates physician health team member and community population outreach activities to support education and awareness of best practice guidelines and program resources.
Experience Requirements:
3 Years Nursing - Related Area Required - And 5 Years Nursing - Related Area Preferred In Clinical - Case Management Or Clinical - Discharge Planning Or Accounting/Finance - Third Party Review Or Clinical - Home or Community Health Preferred
Education Required:
Graduate Nursing And Bachelor's Degree Required - In Nursing Preferred
Licensure/Certification Requirements:
Registered Nurse Required - And Certified Case Manager Or Commission for Case Management Certification Required - Within 1 Year of Hire
Hours of Work:
Monday - Friday, 8am - 5pm generally; 8-10 hours per day with occassional weekend outreach activity
