Position Information:
The Behavioral Health Counselor - Transitional Care Specialist (TCS) a licensed clinical social worker is responsible for providing transitional care services to patients utilizing emergency and crisis services due to a substance use or a mental health disorder. The TCS is responsible for screening all referrals assessing potential patients for appropriateness of transitional care services. TCS is responsible for arranging outpatient services if a different level of care is required. In collaboration with the Peer Support Specialists the TCS facilitates the safe and timely transition from hospital care to community based services. The Transitional Care team consists of two Transitional Care Specialists and three Peer Support Specialists.
Experience Requirements:
3 Years Clinical - Mental Health Or Clinical - Substance Abuse/Addiction Required
Education Required:
Bachelor's Degree Nursing Or Master's Degree Social Work Or Counseling Required - And Master's Degree Nursing Preferred
Licensure/Certification Requirements:
Licensed Clinical Social Worker Required - Or Licensed Clinical Mental Health Counselor Required - Or Licensed Clinical Addictions Specialist Required - And Maintain Vehicle Operator Insurability Required - And Certified Notary Preferred
Hours of Work:
Saturday and Sunday Every Other weekend
