WakeMed Cary Hospital provides exceptional patient care to the residents of Cary, N.C., and the surrounding area. This full-service community hospital has 178 inpatient beds and is part of the WakeMed Health & Hospitals system. The only hospital in westernWake County, Cary Hospital offers an array of services including: surgical services; Women's Pavilion & Birthplace; intensive care; general medical and surgical nursing care; telemetry monitoring unit; cardiac and vascular catheterization labs; centers foradvanced gastrointestinal, chest pain and sleep; inpatient rehab and outpatient specialty rehab services; and outpatient ancillary services. The Ambulatory Care Nurse is a Registered Nurse responsible for providing nursing care management services to assigned populations. Utilizes a collaborative interdisciplinary approach to anticipate, integrate, and coordinate the patient's plan of care and maximize the ability to remain in a community setting. The goal of the intervention is to meet the needs of patients in order to optimize clinical and resource outcomes, and to monitor the effectiveness of the plan and its outcomes. In collaboration with the physician, the nurse identifies at risk patient populations who require care management services. Provides bio-psychosocial assessments to identify patient needs and to assist in the development of the plan of care. The Ambulatory Care Nurse serves as a patient/family advocate and acts as a communication link between the physician, patient, family, outpatient and inpatient health care team, as well as community resources, to reduce redundancy and fragmentation of care.
