Highly skilled, experienced and specially trained staff members who have all the qualifications and medical expertise to deliver professional, competent care to patients. Candidates will have a background in home health, be bonded and insured, able to pass a background check, and have a current license and certification.

Our patients receive care from people who are interested in promoting their comfort and safety. Most importantly, our staff serves as a link between the patient’s physician and family, and keeps the patient informed of his or her progress.

Position Information:
The Licensed Practical Nurse provides direct patient care through personal accountability for care needs of patients and families as demonstrated through treatments, administration of medications and other specified assignment under the direction of a Clinical Nurse. The LPN in Home Health supports the Home Health Plan of Care and collaborates with the interdisciplinary treatment team.   The LPN understands the needs of the organization and supports the mission, values, and management of patient care services.

Experience Requirements:
1 Year Nursing - Related Area Preferred

Education Required:
Graduate Practical Nursing Required

Licensure/Certification Requirements:
Licensed Practical Nurse Required

Hours of Work:
M-F, 8:00am-5:00pm

Weekend Requirements:
As Needed
